Located in central Roseville, Sierra Hills Care Center offers a wide range of medical services. We provide comprehensive nursing and extensive rehabilitative services including IV therapy, wound care, post-operative care, and innovative physical, occupational, and speech therapies. Individualized Plans of Care
At Sierra Hills, our residents' care plans include much more than 24-hour skilled nursing care. The plans of care also address the social, spiritual and emotional needs of residents, their families and friends. The Sierra Hills staff realizes the importance of individualized care plans designed to meet the resident's medical and social needs. These care plans are based on input from our Interdisciplinary Team of physicians, therapists, nurses, social services, dietary, and support staff.
Upon admission, our rehabilitation and skilled nursing teams evaluate residents to determine the course of treatment that will provide optimum results. Our goal is to return each resident to his or her highest level of independence, be it returning home or to a lower level of care. For those who return home, we offer full discharge planning and an evaluation to address safety issues and concerns.
